Patient, Family, or Population Health Problem Solution
A 58-year-old community member was interviewed, gathered statistics, and analyzed literature for the capstone project. High blood pressure was studied through the lenses of leadership, collaboration, openness, and adaptability. Legislation, healthcare quality and safety, cost savings and innovation, and community resources and care coordination were all considered in the search for a cure for hypertension. A PowerPoint presentation was produced to help the patient better understand high blood pressure, the risk factors that contribute to it, the long-term effects it can have, and the treatment options available to him. Leadership and Change Management in Addressing High Blood Pressure Using the efforts of others to achieve one's own ends is a key component of leadership. The role of a leader is to provide direction and organization for others. Leaders who communicate their vision and strategy are successful in change management. Communicative and relationship management skills are essential for leaders.Errida & Lotfi (2021) has shown that a good leadership style involves sharing authority and encouraging involvement for all teams. The patient needed to be educated on the dangers of high blood pressure and the advantages of making minor changes to his lifestyle to improve his overall health. To ensure his success, I included him in his care and chose his own objectives. When it came to treating high blood pressure, I employed Kotter's 8-Step Change Model. The middle-of-the-road theory of change management developed by John Kotter describes and forecasts change. It is possible to test this theory, unlike many other famous theories. In Kotter's theory of change implementation, there are eight basic steps: Create quick wins, reinforce the change, and formalize it by giving people the authority to act on it(Baloh et al., 2017)
